To apply for admission to Knight Academy for 2009-2010, please complete the following

    (To read and print the application, you will need a (free) copy of Acrobat Reader.)
3. In addition, we need the following documents with all ECE-5th grade applications

• Original Birth Certificate
• Current Immunization Record
• Proof of Address – Lease agreement, utility bill, or water bill.  If you share a home with someone, ask them to provide you with one of these documents and ask them to write a note on the document indicating that you reside with them, sign it, and date it.
• Most recent report card or progress report--For students going into the 1^st through 5^th grades.
• ECE and Kindergarten (tuition based) require proof of income – W-2 form or 2 most recent pay stubs.
